  • The person in charge failed in their duties and responsibilities as evidenced by this noncompliant report.
  • Food employees with full facial hair observed involved in food preparation not wearing proper hair restraints such as beard covers.
  • Time in lieu of temperature being used in the food facility to control ready-to-eat, potentially hazardous food (pizza sauce) without written procedures or documentation to verify disposition of food. Corrected on the spot.
  • Durability of materials used as food contact surfaces. Observed in-use pizza pans being stored in cardboard boxes on the floor under the pizza oven. Cardboard is not durable or cleanable.
  • Observed a plastic food service basket being used as a scoop in fresh cut potatoes.
  • Observed an ice scoop being stored directly on top of the bulk ice machine, which is not a clean storage location.
  • Observed in-use utensils (tongs) hanging from deep fat fryer basket handles.
  • Observed packaged (clam shell container) refrigerated desserts not labeled with the common name of the food, ingredients including sub ingredients in descending order including a declaration of artificial colors, flavors, chemical preservatives, accurate declaration of net contents, name and place of manufacture, major allergens if applicable and disclosure of canthaxanthin ore astaxanthin for salmonid fish.
  • Improper ware washing. Employees on duty are washing in hot soapy water then immersing in sanitizing solution then air drying missing the rinse step to completely remove detergent and or food residual.
  • Storage of pans. Observed wire mess style pizza dough pans being stored in cardboard boxes on the floor under the pizza ovens.
  • Paper towel dispenser empty at the handwash sink in the food preparation area.
  • The food facility does not have the original certificate for the certified food employee posted in public view.

    Restaurant comments: One of the owners said the issues have been resolved and the restaurant is awaiting re-inspection.

  • Lack of hot and cold water under pressure due to non functioning generator on this mobile food facility.

  • Shelving and handles on bain-marie non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil.
  • Floor in the walk in cooler is made of either wood or metal that is corroded and is not smooth, non-absorbent, or easily cleanable as required.
  • Buckets filled with raw chicken lined up outside of the walk-in cooler stored open with no covering.
  • Underside of shelf over finished plate pass through, a food contact surface, was observed to have food residue and was not clean to sight and touch.
  • Floor under fryers and cook line equipment is extremely dirty, greasy, and in need of cleaning.

  • Retail food facilities HACCP Plan states that smoked meats are directly moved from the smoker to cooler, but are not following the procedures listed in their own HACCP Plan.
  • Retail food facility processing smoked meats has no records indicating that smoked meats were properly cooled from 135 degrees to 70 degrees in two hours and/or from 135 degrees to 41 degrees within six hours after preparation.
  • Observed smoked meats being packaged using reduced oxygen packaging by this retail food facility that are not properly labeled with the common name of the food, ingredients (including sub ingredients) in descending order of predominance by weight including a declaration of artificial colors, flavors, chemical preservatives, accurate declaration of net quantity, name and place of manufacture, major allergens if applicable and disclosure of canthaxanthin ore astaxanthin for salmonid fish.
  • Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Food facility has test strips for quaternary ammonia, but is using chlorine as a sanitizer.
  • Frequency of cleaning food contact surfaces. Observed dried hardened food debris on the cutting blade and other associated food contact surfaces of the electric meat slicer.
  • Food facility is using an approved non-public water system, but does not have current laboratory testing results for water potability.

  • HACCP related records do not appear organized and not easily audited.
  • Smoked meats packaged using reduced oxygen packaging are not properly date marked with the date of pack and sell by date as required.
  • Food facility is reduced oxygen packaging both smoked and raw meats, which has a potential for clostridium botulinum/listeria monocytogenes, without documenting or monitoring a secondary food safety barrier. All ROP packaging must cease until documentation is submitted for a secondary food safety barrier and a HACCP Plan is submitted and approved by the department.
