An attorney has agreed to discipline by consent over a loan out agreement that was missing language required under the Arizona Rules of Professional Conduct.

The disciplinary case stemmed from an agreement between attorney David T. Glynn and Melanie Kunz, a 73 year-old nurse practitioner, who owns MDTriage, which is a company that provides physicians with off-hours telephone services staffed by nurses, nurse practitioners, and medical assistants, according to the agreement for discipline.
